All donated cargo must be approved by AmericasRelief Team. In order to proceed with your request, the following information is required as a formality/policy in order to ship donated cargo.

Please Answer The Following Questions
  1. Is your organization an IRS 501-c-3 non-profit organization?  
2. Who is the Consignee (organization receiving the cargo) in the receiving country? Please also provide the contact information:
3. Does the Consignee have written authorization/approval to import the container(s) or loose cargo? And, can they assure us that the authorities will not place the cargo on hold?
4. Has the Consignee made arrangements to receive and discharged the container immediately upon arrival and to return the empty container back to the destination?

Please list below a complete breakdown/disclosure of the donated contents/items being shipped and state that the goods have no commercial value, and that they will not be sold for profit in the receiving country.

The above instructions apply to both Full Load Container, and Loose Container Load. You must complete this form and recieve our authorization before the cargo is shipped, otherwise it will be placed on hold, and demurrage charges will apply.
